Please select the Diamond Package only if you are requesting up to six (6) weeks of short-term disability leave.
This package covers a MAXIMUM of six weeks of approved leave. Please note that extensions beyond the initial six-week period are not available. If you anticipate needing additional time beyond six weeks, please select the Ruby Package.

RETURN & REFUND POLICY
- Refunds: Due to the nature of medical clearance and leave paperwork services, all payments are non-refundable once services have been initiated (e.g., chart review, lab/diagnostic orders, paperwork review and completion).
- Cancellations: If you wish to cancel, you must notify us at least 48 hours prior to your scheduled appointment. Cancellations made within 48 hours may be eligible for a refund, minus a $75 administrator fee.
- Provider Rights: The MedClearer reserves the right to cancel or withdraw services if a patient is uncooperative, disrespectful, or fails to provide required documentation. In such cases, only unrendered services may be eligible for refund consideration.

A bundled option for clients who need both STD and FMLA paperwork completed. Includes a TeleHealth Pre-Op & Post-Op Appointment, completion of STD paperwork, completion of FMLA paperwork, and one Return to Work form.
