I Have Obtained My Required Forms
Your employer will provide the specific forms your provider must complete. If applicable, please have these forms before purchasing or your claim number.
I Have Initiated Any Required Insurance or Disability Claim
The MedClearer™ completes medical documentation, but the claim itself is initiated by you.
I Know My Documentation Deadline
Know when completed forms must be submitted so the service timeline matches your needs.
